Under the supervision of the Social Work Supervisor, the Social Worker Assistant assists social workers in providing psychosocial services to participants. General Duties and Responsibilities Advise and guide participants through enrollment in needed services such as Paratransit, Meals on Wheels, housing, and other community services. Visit participants in their homes and provide phone support. Accompany participants to non-medical appointments as needed. Gather materials to file; find and retrieve information from files in response to requests from Social Workers. Communicate with community service organizations for information gathering and act as a liaison for participants. Assist participants with computerized assessments and documents. Administer screening assessments and review care plans with participants. Maintain the confidentiality of all company procedures, results and information about participants, clients or families in conformance with HIPAA principles. Perform related duties as assigned. Qualifications Associate’s Degree from an accredited school. One year of experience in a health care setting (preferred). One year of experience working with a senior population (preferred). Demonstrated ability to work in a multicultural and multidisciplinary team environment. Ability to work with minimal supervision. Excellent oral and verbal communication and meeting facilitation skills. Bilingual ability preferred (Spanish, Korean, Cantonese, and/or Mandarin). Valid California driver’s license with an acceptable driving record and insurance and reliable transportation. Current CPR and First Aid certification.
